This is the ONLY Physical form that will be accepted for participation next year. The only form acceptable is the one that has “Revised April 2007” in a box on the upper left hand corner of the first page. It also has a VHSL emblem on the upper right hand corner of the first page.

2008-2009 Sports Physicals ()
The Hayfield Athletic Training Program will be hosting Sports Physicals on Monday, August 4th 2008 from 6-9 pm. These physicals will offer a comprehensive orthopedic screening given by FCPS Certified Athletic Trainers, Physical Therapists, Registered Nurses, and Team Physicians. All athletes who plan to participate in sports in the upcoming year MUST have a VHSL physical on file in order to participate. To preregister for your physical, please go to www.fcps.edu/supt/activities/atp/ppeschedule.htm. The cost is $40 for pre-registration and $50 for walk-ins.
